Connecting People and Community for Living Well - PHC IGSI

program image
Description: The Connecting People and Community for Living Well Primary Health Care Integrated Geriatric Services Initiative (PHC IGSI) is focused on increasing capacity in community teams to provide ongoing support for people living in the community affected by dementia – including those with a diagnosis as well as their family carers. The Connecting People and Community for Living Well approach includes provincial team support for participating community teams to: build relationships across sectors locally (health care, social and community), identify local resources, intentionally seek to understand the unmet needs of those affected by dementia, work together to design and implement priority activities to address identified needs. The innovations which arise from establishing non-traditional partnerships have enhanced both the supports available, and the communities’ awareness of the impact of living with dementia. PHC IGSI is part of the Alberta Health Services (AHS) which is the health network for Alberta. https://www.youtube.com/watch?v=VpZOXqPmLGs&ab_channel=HealthyAgingCORE
