Community Residential Care Survey: Supporting Mental Health

Do you or a family member/friend live in a Community Residential Care Facility? If yes, please consider taking a few minutes to complete this survey. The Canadian Coalition for Senior’s Mental Health (CCSMH) is conducting a survey to inform a Research Report and Recommendations on Supporting Mental Health and Well-Being in Community Residential Care Settings (including long-term care facilities, assisted living and supportive housing). The purpose of the survey is to incorporate resident and family perspectives on Mental Health and Well-being into our report. Participation in this survey is completely voluntary. Only anonymous testimonials and summary data will be used by the project team. The survey will take you 3-5 minutes to complete.
