[Guide] Community Asset Mapping Guide for Social Prescribing
This guide is intended for link workers (aka community connectors), healthcare providers, community organizations, policymakers, funders, and community members who are working to strengthen social prescribing in Canada. Its purpose is to support more effective referrals, foster collaboration across sectors, inform planning and decision-making, and ensure that social prescribing remains grounded in the strengths and priorities of communities themselves.
