[Report] Shaping the Future of Care Closer to Home for Older Adults

Most older adults want the choice to age in place, living safely and independently at home or in their community. Healthcare Excellence Canada (HEC) is working to make this a reality through its five-year strategic plan. After reviewing research and consulting with stakeholders, HEC found major barriers like fragmented care systems and limited resources. By centering lived experience, they’re identifying solutions to close these gaps and ensure older adults and their care partners have the support they need to thrive at home.
