Social Prescribing in Ontario - Final Report (March 2020)
Eleven Ontario Community Health Centres participated in the the Rx: Community pilot. Using client- and design-centered thinking, they identified non-clinical interventions, built a structured referral pathway, and tracked the impact of their work.
