[Toolkit] Social Isolation and Loneliness – Tools & Resources
Social isolation and loneliness are increasingly recognized as significant health risks for older adults, contributing to poorer mental and physical health outcomes. Health care and social service professionals play a critical role in identifying these risks and supporting older adults in maintaining meaningful social connections.
Many organizations across health care, community services, and the non-profit sector are doing important work to address social isolation and loneliness among older adults. Building on these efforts, continued collaboration and shared learning can further strengthen supports, enhance early identification, and help ensure older adults remain connected to meaningful relationships and resources that promote their overall well-being.
To support this work, CCSMH has developed a suite of practical tools to support the mobilization of the CCSMH Clinical Guidelines on Social Isolation and Loneliness in Older Adults. These resources are designed for use across health care and social service settings:
- Collaborative C.A.R.E. Tool: An interactive framework to support professionals in assessing risk factors, identifying appropriate supports, and connecting older adults with relevant community programs and services.
- Conversation Starter Resource: Provides sample language and guidance to help professionals initiate respectful and supportive conversations about social isolation and loneliness with older adult patients or clients.
- Conversation Starter for Older Adults: A supportive resource that helps older adults begin discussing feelings of loneliness and social isolation, offering simple prompts, self-reflection tips, and guidance to build confidence in seeking support.
- Three-Part Video Series: Designed for health care and social service professionals, this series offers high-level insights and practical guidance on understanding and addressing social isolation and loneliness, including its impacts and clinical approaches. The videos are available on YouTube (part 1, part 2, part 3)
