Breaking Barriers: Building Palliative Care Everywhere

Palliative care is person-centered care. This means that wherever there is a person with a life-limiting illness, palliative care can be provided to help alleviate their suffering. Regardless of the setting of care, illness or prognosis, or even postal code, palliative care can offer comfort and support.


This National Hospice Palliative Care Week, join CHPCA on May 11, from 2:00 PM to 3:00 EST, for Breaking Barriers: Building Palliative Care Everywhere, a showcase of innovative programs and projects working to make palliative care more accessible in communities across Canada.


During the webinar, you will hear from SE Health about their Reflection Room and Journey Home Hospice projects, Nav-CARE - a volunteer program that aims to alleviate social isolation and provide service/community navigation, and Strengthening a Palliative Approach in Long-Term Care (SPA-LTC) - a program that ensures people with life-limiting illnesses have more opportunities to experience a high quality of life while living in long-term care residences.
