With the rise in isolation amidst the COVID-19 pandemic, organizations such as Mosaic and 8 80 Cities have adopted asset-based community development to encourage equitable community engagement and socialization, especially among older people — protecting their right to mobility and providing the ability to age in place. To share their extensive experience in applying a community integrated model of care that addresses the clinical, personal supports, and wider non-clinical psychosocial needs of the person, Ms. Jane Teasdale and Ms. Jayne Armstrong will be leading a discussion on “Person Centered Care & The Seniors' Outdoor Activation Toolkit” in the IFA Global Café series.
